Remdesivir Reduces Mortality in Hospitalized Covid-19 Patients Requiring Low-Flow Oxygen

This poster reports a large retrospective U.S. comparative effectiveness study evaluating whether early remdesivir (RDV) reduces inpatient mortality in hospitalized adults with COVID-19 who required low-flow oxygen (LFO) within the first two days of admission. The study used the PINC AI Healthcare Database and included patients hospitalized from December 2020 through April 2022, spanning three variant-of-concern periods: pre-Delta, Delta, and Omicron.<br /><br />After applying inclusion and exclusion criteria, 116,012 patients on LFO were analyzed. Most received RDV early (81,811), while 34,201 did not. Propensity score matching produced well-balanced groups of 67,582 patients in each arm. The main outcomes were 14-day and 28-day all-cause inpatient mortality.<br /><br />Across the full matched cohort, RDV was associated with significantly lower mortality than no RDV, both in unadjusted Kaplan-Meier analyses and in adjusted Cox proportional hazards models. This mortality benefit was consistent across all three variant periods. The adjusted results showed lower 14-day mortality for RDV versus non-RDV patients (6.4% vs 8.8% overall) and lower 28-day mortality (9.8% vs 12.3% overall), with similar patterns in pre-Delta, Delta, and Omicron periods.<br /><br />The authors conclude that starting RDV within two days of hospital admission is linked to significant reductions in 14- and 28-day mortality among hospitalized COVID-19 patients requiring low-flow oxygen, regardless of circulating variant period. They also note that RDV had shown similar benefit in patients with higher oxygen needs in prior work. Despite guideline support and observed benefits, more than one-quarter of eligible patients did not receive RDV, suggesting room to improve use of evidence-based treatment.
