Relationship of Qi and Work Engagement With Burnout in Hospitalists

This study examined quality improvement (QI) interest and activity among UCLA hospitalists, and how these relate to burnout and work engagement. After the COVID-19 pandemic, burnout among hospitalists was noted to be high, prompting a survey of all 137 faculty members in spring 2022. The survey measured QI interests and involvement, barriers to change, leadership support, confidence in doing QI work, burnout, and work engagement using the Utrecht Work Engagement Scale.<br /><br />The response rate was 48%, with most responses from main campus sites. Hospitalists at the main campuses and community sites did not differ in whether they could identify opportunities for improvement, but main campus hospitalists were more likely to report having opportunities to make improvements. Confidence in the ability to design and implement QI projects was moderate overall (3.1/5) and did not differ between groups.<br /><br />The key findings suggest that community hospitalists had fewer opportunities to participate in improvement projects, not because they lacked ideas or confidence, but likely because of differences in workplace culture and environment. The study also found that faculty without burnout had higher work engagement. However, involvement in QI projects and hospital committees may be influenced more by internal motivation than by burnout or engagement alone.<br /><br />Overall, the authors conclude that successful QI efforts should consider feasibility, impact, and engagement from end users. Stakeholder alignment is essential, especially in the current healthcare environment where hospitals are under pressure to reduce length of stay and costs.
