Regional Differences Between Northeast and Southern Hospitalizations for Patients Admitted With Chronic Obstructive Pulmonary Disease

This retrospective cohort study examined regional differences in hospital outcomes for patients admitted with chronic obstructive pulmonary disease (COPD) in the Northeast versus the South from 2016 to 2019. Using the National Inpatient Sample and monthly temperature data from the National Centers for Environmental Information, the authors compared admissions and outcomes across these U.S. regions and assessed whether temperature was related to monthly COPD admissions.<br /><br />A total of 463,830 COPD admissions occurred in the Northeast (30.1%) and 1,078,930 in the South (69.9%). In both regions, the lowest hospitalization rates were seen during the hottest months, July and August. The study found that patients hospitalized in the Northeast had worse in-hospital outcomes than those in the South: higher odds of death, lower likelihood of discharge home, longer lengths of stay, and higher hospital charges.<br /><br />The authors conclude that there are significant regional disparities in COPD outcomes, with the Northeast faring worse across all in-hospital measures. They suggest that colder temperatures may contribute to these differences and emphasize the need for healthcare systems to be especially vigilant in managing COPD patients during colder periods. Further research is recommended to better understand the environmental and other root causes of these regional disparities.<br /><br />A key limitation is that the data were aggregated by region and month, so conclusions about individual patients cannot be made.
