Regarding Risk Factors and Outcomes in Covid-19 and Myocardial Infarction

This study examined whether COVID-19 affected outcomes in patients hospitalized with acute myocardial infarction (AMI) in the United States during 2020, using the National Inpatient Sample. Among 567,990 AMI admissions, propensity matching identified 1,154 patients with COVID-19 and 109,990 without COVID-19. The groups were similar in sex distribution, age, and overall comorbidity burden, but patients with COVID-19 were less likely to have several traditional AMI risk factors, including dyslipidemia, peripheral vascular disease, smoking, hypertension, and carotid artery disease.<br /><br />Despite having fewer classic cardiovascular risk factors, the COVID-19 group had significantly worse in-hospital outcomes. After adjustment, COVID-19 was associated with higher hospital mortality, more cardiogenic shock, more respiratory failure, and more cardiac arrest. COVID-19 was also linked to longer hospital stays and increased hospitalization charges.<br /><br />The authors conclude that AMI patients with concurrent COVID-19 experience substantially worse outcomes than AMI patients without COVID-19. They suggest that this high-risk subgroup may require heightened clinical suspicion, rapid recognition, and protocol-driven management to improve outcomes.
