Refeeding Syndrome Occurs More Frequently in Frail Patients

This retrospective single-center study examined whether frailty predicts refeeding syndrome in hospitalized adults with acute medical problems. The authors analyzed 201 patients aged 20–90 years discharged from a tertiary hospital’s acute medical unit. Frailty was defined by a Clinical Frailty Scale (CFS) score of 5 or higher. Refeeding syndrome was defined as a ≥10% drop in serum phosphorus or potassium within 5 days of admission among patients with reduced oral intake before hospitalization or those receiving parenteral nutrition.<br /><br />The frail group (103 patients) was older, weighed less, and had more comorbid illness and inpatient fasting than the non-frail group (98 patients). Refeeding syndrome occurred much more often in frail patients than in non-frail patients (44.7% vs. 18.4%). In univariate analyses, higher CFS, lower weight and BMI, fasting after admission, higher comorbidity burden, peptic ulcer disease, low potassium, low protein/albumin, and elevated hs-CRP were associated with refeeding syndrome.<br /><br />After multivariable adjustment, three factors remained independently associated with refeeding syndrome: higher frailty score (OR 1.31 per 1-point increase), hypokalemia at admission (OR 3.19), and fasting after admission (OR 3.91). Refeeding syndrome was also linked to worse outcomes, including longer hospital stay (21.5 vs. 14.2 days) and more pressure ulcers (15.6% vs. 3.6%).<br /><br />Overall, the study suggests that frailty, as measured by the Clinical Frailty Scale, is an important predictor of refeeding syndrome in hospitalized medical patients.
