Reduction of Hospital-Acquired Venous Thromboembolism Through Document Standardization

The project addressed poor documentation of venous thromboembolism (VTE) prophylaxis orders in hospitalized patients, a problem that can contribute to hospital-acquired VTE. Since manual note checking and copy-pasted notes often led to inaccurate records, the team created a standardized Epic SmartText that automatically populated key clinical information, including VTE prophylaxis, code status, diet order, health care proxy, and emergency contacts. This SmartText was embedded into a standardized note template for use on the Bellevue medicine service.<br /><br />The intervention was evaluated by comparing note accuracy and hospital-onset VTE rates before and after implementation. Before the SmartText, note accuracy was 68%. After six months of use, accuracy improved to 92%, a 24% increase. In parallel, the hospital-acquired VTE rate decreased from 0.311 events per 1000 patient days to 0.225 events per 1000 patient days, representing a 28% reduction.<br /><br />The findings suggest that standardized documentation with practical SmartTexts is an easy-to-implement quality improvement strategy that can improve documentation accuracy and may also reduce hospital-acquired VTE. The authors plan to expand the SmartText and note template across the NYC Health + Hospitals system and consider similar standardized documentation approaches for other hospital-acquired conditions.
