Reducing Revisits: An Evaluation of the Enrollment and Onboarding Process of 21 U.S. Hospitals

The REVISITS study evaluated the enrollment and onboarding process for a multi-site COPD transition-of-care quality improvement trial involving 21 U.S. hospitals. Funded by the NIH and launched in 2020, the study used implementation science methods in a type II hybrid effectiveness-implementation cluster randomized clinical trial. Because hospitals—not individual patients—were the study subjects, recruitment and onboarding required a nontraditional site-level process.<br /><br />Recruitment ran from September 2021 to December 2022 through HOMERuN and the Society of Hospital Medicine. Of 95 sites expressing interest, 57 submitted applications, 42 began enrollment, and 21 ultimately enrolled. Site retention was high, with 20 of 21 sites remaining in the study. To support sites, the team held onboarding sessions for all site leads and completed virtual mentoring sessions with each site.<br /><br />Evaluation data came from REDCap surveys completed by 41 of 44 site leads (93%). Results showed strong satisfaction with the process: all respondents felt supported during enrollment, and all found the individual virtual onboarding sessions helpful. Most said expectations were clear. However, some sites experienced difficulty obtaining required approvals and documentation, especially the data administrator letter of support, which 62% found difficult to secure. By contrast, fewer site leads reported challenges with IRB approval, QI determination, or data use agreements.<br /><br />Overall, the findings highlight both the strengths and barriers of hospital-site enrollment. The study underscores the importance of providing structured support, clear communication, and tailored onboarding to help hospitals successfully join and remain engaged in multi-site implementation research.
