Reducing Length of Stay and Enhancing Discharge Efficiency Using a Double Emergency Department Admitter Model at a Tertiary Care Hospital.

This poster describes a quality-improvement intervention at INTEGRIS Health Baptist Medical Center in Oklahoma City: the introduction of a Double Emergency Department Admitter (DEDA) model. In this workflow, two hospitalist physicians were dedicated to ED admissions during peak hours so that rounding hospitalists could focus more on inpatient care and discharges.<br /><br />The study used a retrospective before-and-after analysis to assess the effect of the DEDA model on hospitalist and ED throughput metrics. Hospitalist outcomes included length of stay (LOS), observed-to-expected LOS ratio, geometric mean LOS (GMLOS), percentage of discharges by noon, and monthly discharge volume. ED outcomes included the percentage of admitted patients remaining in the ED longer than 120 and 280 minutes after an admit order, time from admission decision to ED departure, and total ED LOS for admitted patients. SPSS was used for descriptive statistics, with p < 0.05 considered significant.<br /><br />Results showed improved hospitalist performance after implementation. LOS decreased from 6.7 to 6.2 days, LOS O/E improved from 1.57 to 1.47, GMLOS decreased slightly from 4.29 to 4.19 days, discharges by noon rose from 13.5% to 15.2%, and monthly discharges increased from 1,287 to 1,493. ED metrics also improved, including fewer patients boarding in the ED, a shorter time from admission decision to ED departure, and reduced total ED LOS for admitted patients.<br /><br />Overall, the poster concludes that a dedicated admitting team during peak ED hours can improve inpatient efficiency, reduce ED boarding, and enhance patient flow and resource utilization in a high-volume tertiary care setting.
