Redesign and Evaluation of a Perioperative Medicine Curriculum

This study describes a redesign of a perioperative medicine curriculum for internal medicine residents at the University of Texas at San Antonio, who rotate between University Hospital and the Audie L. Murphy Veterans Hospital. The original curriculum relied on daily faculty-led literature review sessions and a printed “Bluebook” of selected articles. Using Kern’s 6-step curriculum design framework, the program was updated to better meet modern training needs and incorporate standardized learning resources.<br /><br />The redesign included updating the Bluebook with newer perioperative medicine articles, adding Society of Hospital Medicine online modules, continuing daily faculty-led critical appraisal, and introducing weekly “Module Fridays.” Teaching topics were also assigned to specific days to create a more structured learning plan.<br /><br />To evaluate the curriculum, residents completed surveys at the start and end of their 4-week rotation from July 2023 to June 2024. Surveys measured confidence in perioperative topics including medication management, preoperative testing, and chronic conditions such as hypertension, diabetes, anemia, liver disease, coronary artery disease, and valvular heart disease, as well as complications like acute kidney injury, urinary retention, delirium, and venous thromboembolism prophylaxis.<br /><br />Results showed improved self-reported confidence across all 42 survey questions, with statistically significant gains in 15 questions. Although the study was limited by a low response rate and was underpowered to show significance for every item, residents reported better understanding of many key perioperative medicine topics.<br /><br />Overall, the redesigned curriculum was well received and appeared to strengthen resident confidence and knowledge in perioperative management.
