Red Blood Cell Transfusion Alerts: Guideline Adherence and Clinical Impact

This review examined 32 published manuscripts describing red blood cell (RBC) transfusion clinical decision support (CDS) tools to see how well they aligned with evidence-based transfusion practice. The authors noted that hospitals often use electronic CDS to improve transfusion ordering, but these tools can be difficult to program correctly because transfusion thresholds vary by patient condition and guidelines also emphasize clinical context, not hemoglobin alone.<br /><br />The review assessed whether studies justified CDS use with trial data or guidelines, whether the CDS matched those recommendations, whether the CDS relied only on hemoglobin/hematocrit triggers, whether clinician override was allowed, whether the target patient population was described, whether plans for updating the CDS were included, and what outcomes were measured.<br /><br />Key findings showed substantial variation and incomplete adherence to best practices. Only 37% of studies cited transfusion guidelines or trial data, while 50% used their own institutional policy and 13% provided no justification. Among studies that cited guidelines or trials, only 8% were completely consistent with the evidence, while 84% were partially consistent. More than half of the CDS tools (56%) were based only on a hemoglobin/hematocrit threshold, without incorporating clinical context or comorbidities. Only 50% described the patient population, and just 31% described a plan for updating the CDS as new evidence emerges.<br /><br />Outcomes most commonly measured included red blood cell orders, single- versus multi-unit transfusions, pre- and post-transfusion hemoglobin levels, and transfusions outside recommended guidelines. Fewer studies examined mortality, readmissions, length of stay, or costs.<br /><br />Overall, the authors concluded that many RBC transfusion CDS tools do not accurately reflect current evidence or guidelines, which may limit their ability to promote evidence-based transfusion practice. They recommend frequent review and updating of CDS tools using the latest transfusion guidelines.
