Recurrent Unprovoked Deep Venous Thrombosis With a Coincidental Finding of Bilateral Pulmonary Embolism

This case report describes a 61-year-old woman with a history of prior DVT treated with Xarelto who presented with acute left leg pain, swelling, warmth, and erythema after sitting for several hours. She was also found to be hypoxic and hypertensive on arrival. Imaging revealed an extensive occlusive thrombus in the left distal superficial femoral and popliteal veins, along with bilateral subsegmental pulmonary emboli without right heart strain.<br /><br />Her medical history included coronary artery disease with prior stents and CABG, hypertension, hyperlipidemia, GERD, HFpEF, peripheral vascular disease, and right below-knee amputation. Family history was notable for recurrent thrombosis in her mother. Laboratory studies were largely unremarkable, and a hypercoagulability evaluation was negative.<br /><br />She underwent mechanical thrombectomy with removal of a large clot and was treated with therapeutic Lovenox for one month, then transitioned to Eliquis and discharged safely. The authors emphasize that recurrent unprovoked DVT, especially in a patient with family history and no clear provoking factor, carries a high risk of recurrence and typically warrants indefinite anticoagulation. They also highlight that when an unprovoked DVT is suspected, a hypercoagulability workup should be initiated promptly before anticoagulation when appropriate.
