This document describes focal nodular hyperplasia (FNH), a common benign liver mass seen in adults, with a prevalence of about 0.3–3%. FNH is thought to arise as a hyperplastic response to vascular abnormalities and is often found incidentally on imaging. It is usually asymptomatic, but larger lesions can cause abdominal pain, especially when they exceed 10 cm. FNH is more common in women aged 20–50, which has led to interest in a possible hormonal influence, although the relationship with oral contraceptives remains controversial and recent studies have not shown a clear association with incidence, size, or number of lesions.<br /><br />The case report focuses on a 31-year-old woman who developed recurrent right upper quadrant pain after pregnancies. Imaging revealed a 7 × 6 cm FNH lesion. She declined surgery and instead underwent bland embolization by interventional radiology, which led to symptom resolution. Her symptoms and lesion recurred with later pregnancies, including one ending in miscarriage and another full-term pregnancy, and each time embolization again relieved her discomfort. The lesion size decreased over time, but pregnancy-related recurrence of symptoms persisted.<br /><br />The authors suggest that this case supports a possible link between estrogen, pregnancy, and FNH growth or symptom recurrence, though the evidence remains uncertain. They propose that embolization may be a useful option for symptom control in women with recurrent pregnancy-associated FNH, and they emphasize the need for further research into hormonal influences on this condition.
