Readmission Risk After Covid-19 Hospitalization: A Moderation Analysis by Vital Signs

This study examined 2,557 COVID-19 hospital admissions among 2,230 adults in a Rhode Island health system from April to December 2020 to identify factors linked to 30-day readmission after discharge. Researchers reviewed demographics, comorbidities, hospital course, treatments, and vital signs within 24 hours of discharge, and used moderation analysis to see whether discharge vital signs changed the effect of other risk factors.<br /><br />The sample included many older adults, with nearly half over age 65, and about one-third identifying as Hispanic. Readmission risk was higher in patients with pulmonary disease and certain other clinical risk factors, while cardiovascular disease was also associated with readmission patterns in the analysis. Most importantly, having normal vital signs within 24 hours of discharge was associated with a lower chance of being readmitted within 30 days.<br /><br />The moderation analysis suggested that normal vital signs at discharge were especially protective for patients with higher-risk conditions, including pulmonary disease and psychiatric/substance use disorders. The authors concluded that stable, normalized vital signs before discharge may be particularly important for patients with behavioral health conditions, immunocompromised status, or pulmonary disease.<br /><br />Overall, the study suggests that discharge readiness should not rely on timing alone; clinicians may reduce readmissions by ensuring vital signs have normalized before sending higher-risk COVID-19 patients home.
