Rare Case of Cutaneous Plasmacytoma

This case describes a rare cutaneous manifestation of multiple myeloma in a 72-year-old man. Multiple myeloma is a plasma cell cancer that can cause immune suppression, bone disease, kidney failure, and neurologic problems. Although extramedullary plasmacytomas can develop during the disease course, skin involvement is uncommon.<br /><br />The patient had been followed since 2003 for smoldering myeloma and was later diagnosed with multiple myeloma in 2021 after bone marrow biopsy showed 98% plasma cells. About one year later, he developed nodular lesions on the left occipital scalp and right shoulder. Skin biopsy confirmed these lesions as plasmacytoma.<br /><br />While awaiting treatment, chemotherapy was interrupted for several months because of pneumonia and soft tissue infections. During this pause, the lesions enlarged rapidly, with the right shoulder mass becoming fungating and additional spread to the anterior chest. After chemotherapy was restarted, the skin lesions improved quickly, with some nearly resolving within two weeks.<br /><br />The authors note that cutaneous plasmacytoma is rare and can be distressing because of its appearance, rapid growth, and need for wound care. They found only two previous case reports describing skin findings in multiple myeloma. This case emphasizes the importance of recognizing unusual skin lesions as possible manifestations of myeloma and starting treatment promptly, since these lesions may respond rapidly to chemotherapy.
