Racial Disparities in Systemic Sclerosis :Short-Term Outcomes Study

This retrospective study examined racial and gender disparities in short-term outcomes among patients with systemic sclerosis (SSc) using the U.S. National Inpatient Sample from 2012–2019. Among 52,500 hospitalized adults with SSc, 7,553 had an admitting diagnosis of acute coronary syndrome (ACS).<br /><br />The authors found that ACS was more common in males than females with SSc (19.98% vs 13.33%). By race, Caucasians had the highest ACS incidence, followed by Asians. African Americans underwent percutaneous coronary intervention (PCI) less often than other racial groups.<br /><br />Among SSc patients admitted with ACS, in-hospital mortality differed significantly by race. Asians had the highest adjusted risk of death compared with Caucasians (adjusted HR 1.99), followed by African Americans (adjusted HR 1.21). Additional factors associated with increased mortality included pulmonary hypertension, end-stage renal disease (ESRD), malignancy, and acute kidney injury (AKI). AKI was the strongest predictor of mortality.<br /><br />The study also noted differences in overall outcomes between men and women, including age, length of stay, and mortality. However, gender was not an independent predictor of mortality in the multivariable model.<br /><br />Overall, the findings suggest that the incidence of ACS in SSc varies by sex and race, with males and Caucasians experiencing the highest ACS incidence, while Asians and African Americans with SSc who are hospitalized for ACS face higher in-hospital mortality. The authors conclude that these results demonstrate important racial disparities in outcomes among patients with systemic sclerosis.
