Racial Disparities in Spetic Shock Outcomes

This retrospective study examined racial disparities in septic shock outcomes using the U.S. National Inpatient Sample (NIS) from 2016–2020, including 2,789,890 patients. The cohort was predominantly White (67.5%), followed by Black (14.4%), Hispanic (10.9%), Asian/Pacific Islander (3.3%), and Native American (0.8%); most patients were around 70 years old.<br /><br />After adjusting for confounders, non-White patients generally had worse outcomes than White patients. Mortality odds were higher for Black, Hispanic, Asian/Pacific Islander, and Native American patients. Minority groups also had increased use of vasopressors, mechanical ventilation, hemodialysis, and blood transfusions, along with higher rates of complications such as acute kidney injury, myocardial infarction, ARDS, cardiac arrest, and acute liver failure. In addition, several non-White groups had longer hospital stays and higher inflation-adjusted hospital charges. Palliative care consultation was used less often in many minority groups.<br /><br />The study concludes that racial disparities in septic shock remain substantial and multifactorial, reflecting broader healthcare inequities. It calls for equitable interventions, larger uniform studies, and deeper research into the underlying causes of these differences. Limitations include reliance on ICD-10 administrative coding, lack of detailed clinical severity data, and limited patient-level information in the NIS database.
