RBT-1 Reduces Post-Operative Anemia, Blood Transfusion, and Iron Use

This randomized, double-blind, placebo-controlled Phase 2 study evaluated RBT-1, a preoperative pharmacologic preconditioning agent, in 121 patients undergoing non-emergent CABG and/or cardiac valve surgery with cardiopulmonary bypass. Patients received a single IV dose of RBT-1 or placebo 24 to 48 hours before surgery. The study tracked post-operative anemia, blood transfusion use, iron supplementation, and hemoglobin levels from baseline through hospital discharge.<br /><br />RBT-1 is a combination of stannic protoporphyrin (SnPP) and iron sucrose (FeS). It is designed to activate anti-inflammatory, antioxidant, and iron-scavenging pathways, potentially improving iron utilization and reducing erythropoietin resistance. Because cardiac surgery commonly causes anemia through blood loss, inflammation, and hemolysis, reducing anemia and transfusion needs could improve outcomes.<br /><br />Results showed that patients treated with RBT-1 had a statistically significant reduction in post-operative anemia compared with placebo. RBT-1 was also associated with fewer blood transfusions and less iron supplementation, including a significant reduction in patients needing both transfusion and iron. Despite these differences in treatment requirements, hemoglobin levels at discharge were similar in both groups. Baseline characteristics and comorbidities were generally balanced between groups.<br /><br />Overall, the findings suggest that preoperative treatment with RBT-1 may reduce post-operative anemia and decrease the need for blood transfusion and supplemental iron in cardiac surgery patients.
