Quality Indicators and Team Composition in Outpatient Parenteral Antimicrobial Therapy Programs: A Scoping Review

This scoping review examined whether reporting more OPAT quality indicators is associated with lower hospital readmission rates, and what multidisciplinary team members commonly participate in OPAT programs.<br /><br />The authors searched PubMed, Embase, Cochrane CENTRAL, Web of Science, and Google Scholar through May 1, 2025. They included adult OPAT studies with multidisciplinary teams that reported readmission rates, excluding conference abstracts. Studies were classified as having low or high readmission rates based on a 10% threshold. Data were extracted on readmissions, team composition, and whether each of 12 proposed OPAT quality indicators was reported.<br /><br />Eighteen studies involving 5,027 patients met criteria. The median readmission rate was 11.3% (IQR 8–20). All included studies described a structured OPAT program and formal team. Reporting more quality indicators did not correlate with fewer readmissions: studies reporting more indicators (median 7) did not have significantly lower readmission rates than those reporting fewer (median 6; p=0.64). However, structured initial patient assessment by a competent team member was more common in lower-readmission studies (100% vs. 70%), though this was not statistically significant.<br /><br />Organization and Initiation indicators were reported more often than Continuation and Outcome indicators. Every program included an infectious diseases physician; most included nurses (94%) and about half included pharmacists (55%). In contrast, only 28% included social workers or case managers, and 11% included hospitalists.<br /><br />The review concludes that structured initial eligibility assessments may be linked to lower readmissions, while stronger involvement of hospitalists and social workers could improve care transitions and support. Future research should evaluate how Continuation- and Outcome-based indicators affect patient safety and efficiency.
