Quality Improvement Initiative to Improve Helicobacter Pylori Eradication Rate After Endoscopic Diagnosis of Gastroduodenal Ulcers.

This quality improvement project examined whether testing and treatment for <em>Helicobacter pylori</em> could be improved after diagnosis of gastroduodenal ulcers (GDU) at a safety-net hospital in Houston. Since <em>H. pylori</em> is a common cause of gastric and duodenal ulcers, guidelines recommend testing, eradication treatment, and confirmation of cure. Using endoscopy records, the team identified 198 patients with GDU between August 2020 and August 2021. The cohort was diverse, with many patients being Black or Hispanic. Overall, <em>H. pylori</em> prevalence was 40.7%. Root cause analysis showed key barriers: limited provider awareness about the need to confirm eradication, limited patient awareness of the importance of treatment, and poor patient engagement due to language barriers and difficulty collecting stool tests. To address these issues, the intervention included provider education on <em>H. pylori</em> testing and effective treatment regimens, plus patient letter templates in English, Spanish, and Vietnamese to improve communication. Results showed improvement in treatment quality. Among patients with <em>H. pylori</em>-associated GDU, treatment rates increased from 88.4% pre-intervention to 95.0% post-intervention, though this was not statistically significant. Confirmation of eradication remained low and did not improve meaningfully (29.3% vs 27.0%). However, prescription of efficacious <em>H. pylori</em> regimens improved significantly from 70.4% to 93.8%. The authors conclude that the intervention improved use of effective treatment regimens, but further efforts are needed to increase eradication confirmation. Future plans include electronic medical record order sets to support testing, treatment, and cure confirmation.
