Prucalopride Enhances Feeding Tolerance in Hospitalized Patients: Retrospective Analysis

This retrospective study examined whether prucalopride could improve feeding intolerance in hospitalized patients with delayed gastric emptying or poor response to prior prokinetics. Prucalopride, a 5-HT4 receptor agonist used for constipation, increases gastrointestinal motility, but its role in feeding intolerance had not been well studied.<br /><br />Researchers reviewed patients treated with prucalopride at Inha University Hospital in Korea from February 2018 to January 2023. Among 1,002 hospitalized patients who received prucalopride, 137 met the study criteria after excluding those with tube feeding, outpatient prescriptions, or other non-relevant cases. The median age was 69 years, and most patients received 1 mg daily. Many had already been treated with other prokinetic agents such as DA-9701, metoclopramide, mosapride, and others.<br /><br />The study found that oral intake improved after prucalopride administration. Median oral intake increased from 845.96 mL the day before treatment to 919.10 mL on the day of treatment, 929.39 mL one day after, and 981.86 mL two days after treatment, with statistically significant differences. Defecation frequency also increased, rising from 0.82 the day before treatment to 1.51 on the treatment day, then remaining elevated over the next two days.<br /><br />Overall, the findings suggest that salvage therapy with prucalopride may help improve feeding tolerance and support nutrition delivery in hospitalized patients with feeding intolerance. However, the authors emphasize that larger, multicenter prospective studies are needed to confirm these results.
