Prognostic Factors and Treatments for Adult Rhabdomyosarcoma of the Extremities

This poster examines prognostic factors and treatments for adult rhabdomyosarcoma (RMS) of the extremities using SEER registry data from 2000–2020. Because adult RMS is rare and has poorer outcomes than pediatric RMS, the authors aimed to identify demographic, socioeconomic, and treatment-related factors associated with survival.<br /><br />The study included 213 adults with upper- or lower-limb RMS. The average age was 64 years, most patients were male, non-Hispanic white, and had income above 200% of the federal poverty level. Most tumors arose in the lower limb. Common treatments included combined chemotherapy, radiation, and surgery; radiation plus surgery; or surgery alone.<br /><br />Kaplan-Meier analysis showed significant survival differences between treatment groups, but not by race/ethnicity, sex, or income. In multivariable Cox regression, all treatment approaches studied were associated with lower mortality risk compared with no treatment. The strongest protective effects were seen with radiation plus surgery and combined chemotherapy-radiation-surgery. Each additional year of age slightly increased mortality risk.<br /><br />The authors conclude that age and treatment modality significantly influence outcomes in adult extremity RMS. Their findings support surgery as the mainstay of treatment, with radiotherapy used as an adjunct, especially for residual or unresectable disease. By contrast, chemotherapy did not appear to improve survival in this adult population, unlike its established role in pediatric RMS.<br /><br />Limitations include the retrospective design, small sample size, and missing SEER data on tumor size, histology, and detailed systemic therapy. Even so, this appears to be the largest population-based survival study of adult extremity RMS to date.
