Prognostic Ability of Cellular Host Response Test in Diagnosis and Risk Stratification of Patients With Suspected Infection in Need of Icu-Level Care

This study evaluated whether need for ICU-level care is a good way to identify sepsis risk in emergency department patients with suspected infection, compared with the Cytovale IntelliSep cellular host response test.<br /><br />The IntelliSep test analyzes leukocyte biophysical properties from a blood sample and produces an IntelliSep Index (ISI) in about 10 minutes, placing patients into one of three risk bands. The investigators pooled data from multiple prospective observational studies across U.S. centers and analyzed 730 adults with complete ICU-level care data. Sepsis status was determined later by blinded physician adjudication.<br /><br />Key findings:<br />- ICU-level care did not closely match sepsis diagnosis or sepsis severity.<br />- Many septic patients were in the non-ICU group, and a meaningful fraction of sepsis-related deaths also occurred among patients not requiring ICU-level care.<br />- IntelliSep risk bands showed a clear stepwise increase in sepsis and mortality rates across Band 1 to Band 3, regardless of ICU status.<br />- No patients in Band 1 died from sepsis in either the ICU or non-ICU groups.<br />- Diagnostic performance remained strong in both ICU and non-ICU cohorts, with high sensitivity and specificity.<br /><br />The authors conclude that ICU-level care alone is not an effective proxy for sepsis risk. In contrast, IntelliSep appears to provide more actionable information by reflecting immune dysregulation directly and may better support early identification and risk stratification of potentially septic patients.
