Preventing Central Line Associated Bloodstream Infection (Clabsi) at Sanford Health Fargo

Sanford Health-Fargo launched a rapid-cycle quality improvement project in January 2021 to reduce central line associated bloodstream infections (CLABSIs), after experiencing 40 CLABSIs in 2020 and a high rate of femoral line use. Because femoral lines carry a higher infection risk than subclavian or internal jugular lines, the team focused on improving central line management practices.<br /><br />A multidisciplinary group of physicians, nurses, quality improvement staff, and infection prevention specialists created an educational intervention for providers and nurses. The session emphasized evidence-based CLABSI prevention, central line placement, and line-care practices, and was offered three times with both in-person and online participation options. The program also provided CME and ANCC credit. To guide the intervention, the team reviewed charts from 80 patients with femoral-line-related CLABSI in 2019 and 2020 to identify opportunities for improvement in line utilization, placement technique, and maintenance. Pre- and post-session surveys measured changes in knowledge.<br /><br />The project was associated with a notable reduction in infections. In 2021, Sanford Health-Fargo recorded 9 CLABSIs compared with 40 in 2020. The average time between CLABSIs increased from 9 days in 2020 to 47 days in 2021. Femoral line utilization also declined, decreasing to 8% per month in 2021 from an average of 15% per month in 2020.<br /><br />Overall, the educational and multidisciplinary QI approach appeared to improve clinician awareness and reduce femoral line use, contributing to fewer CLABSIs at Sanford Health-Fargo.
