Prevalence of Peripheral Intravenous Catheter Associated Bacteremia

This retrospective chart review examined hospital-acquired bloodstream infections caused by coagulase-negative Staphylococcus (CoNS) and Staphylococcus aureus (SA) at a 900-bed urban academic medical center during 2019, with a focus on whether peripheral venous catheters (PVCs) are an underrecognized source compared with central venous catheters (CVCs).<br /><br />The study included blood cultures drawn at least 48 hours after admission. Researchers classified infections as CVC-BSI, PVC-BSI, suspected PVC-BSI, other, or unknown. A suspected PVC-BSI was defined as bacteremia with no other identified source plus a documented PVC complication within 7 days. Only CoNS cases with 2 of 2 positive cultures were included.<br /><br />Among 32 hospital-acquired CoNS bloodstream infections, 10 (32.3%) were attributed to CVCs, 2 (6.5%) to PVCs, and 3 (9.7%) were suspected PVC-related. Among 60 SA bloodstream infections, 7 (11.7%) were CVC-related, 5 (8.3%) PVC-related, and 4 (6.7%) suspected PVC-related. When PVC and suspected PVC cases were combined, the proportion of infections linked to PVCs was similar to that linked to CVCs for both CoNS and SA infections, with no statistically significant difference.<br /><br />The authors conclude that PVC-associated bloodstream infection is likely underrecognized, especially for CoNS bacteremia, and may contribute to a burden similar to that of central line infections. They also note that SA PVC-BSIs can cause significant morbidity. The findings suggest clinicians should maintain a high suspicion for PVCs as the source of hospital-acquired bacteremia and improve PVC care through better nursing practices, dressing protocols, injection safety, and standardized documentation.
