Preliminary Results From a Pediatric to Adult Inpatient Transition Consult

This project evaluated a pediatric-to-adult inpatient transition consult (ITC) designed to support hospitalized patients with childhood-onset chronic conditions and their caregivers as they prepare for adult care. The consult was created because the transition from pediatric to adult healthcare is often stressful and can leave patients and families unprepared.<br /><br />The study used a mixed-methods design to understand patient and caregiver experiences with the ITC and its impact. Eligible participants were English-speaking patients age 18 and older. Data collection included demographics, an adapted survey, and semi-structured interviews focused on transition experiences. Interview transcripts were inductively coded, themes were identified by group consensus, and descriptive statistics were used to summarize survey data.<br /><br />Preliminary findings suggest the ITC was generally well received. Patients and caregivers reported that the consult helped clarify expectations for the adult care model and reinforced or built skills needed for transition, especially self-advocacy. Many participants also described a positive emotional effect from the consult. However, concerns remained, particularly about how adult-care physicians might approach care and whether continuity would be maintained during the transition.<br /><br />The study concludes that inpatient transition counseling may be a useful way to improve readiness for adult care, while also highlighting the need to address lingering worries about adult-provider attitudes and continuity. Next steps include creating written transition plans, including Spanish-speaking patients, conducting follow-up interviews, and tracking outcomes of referrals made after the ITC.
