Pre-Clerkship Medical Student Shadowing on Direct-Care Services Increases Student Understanding and Interest in Hospital Medicine but Disrupts Clinical Workflow

The poster describes the creation and pilot evaluation of the Hospital Medicine Shadowing Experience (HMSE), a structured shadowing program for pre-clerkship medical students at the University of Chicago. The goal was to give students early exposure to hospital medicine on direct-care services (HM-DCS) while supporting professional identity formation through knowledge gain and positive role modeling.<br /><br />Students were paired one-to-one with hospitalist attendings and shadowed for at least two hours on a Sunday using broad, non-prescriptive guidelines. After the experience, students and hospitalists completed anonymous optional surveys to assess satisfaction, feasibility, and perceived disruption to workflow.<br /><br />Results showed strong benefits for students: 100% reported better understanding of hospital medicine practice, 90% improved understanding of non-clinical hospital medicine roles, and 40% became more interested in pursuing hospital medicine as a career. Student satisfaction was very high, with 100% satisfied overall and with role modeling, 90% satisfied with teaching, and 60% satisfied with clinical skills engagement.<br /><br />Hospitalists also viewed the experience positively in terms of teaching: 86% were satisfied with the opportunity to teach, and 71% were satisfied overall. However, all hospitalists reported disruption to HM-DCS workflow, and 29% reported disruption in patient care.<br /><br />The authors conclude that HMSE offers meaningful educational value and may support professional identity formation and educator satisfaction, but it also introduces workflow challenges on already demanding services. Future work should measure disruption more objectively and refine the program to increase student clinical participation while minimizing impact on hospitalists and patient care.
