Post-Discharge Outcomes in Patients on Anticoagulation at the Time of Major Trauma

This retrospective Mayo Clinic study examined 1,056 trauma patients who were taking anticoagulants at hospital admission between 2012 and 2021. The researchers compared outcomes for patients discharged on anticoagulation versus those discharged without it, with follow-up extending to one year after discharge.<br /><br />The patient population was elderly overall (mean age 77.4 years), with 55% male and 45% female patients. Warfarin was the most common anticoagulant, and atrial fibrillation was the leading indication for therapy. Nearly half of the patients (46.9%) did not leave the hospital on anticoagulation despite having an indication for it.<br /><br />Key findings showed no significant difference in overall major bleeding after discharge between the anticoagulation and no-anticoagulation groups. However, the no-anticoagulation group had a significantly higher rate of intracranial bleeding than those discharged on anticoagulation. Most importantly, patients discharged on anticoagulation had significantly fewer cerebrovascular events (CVA/TIA) within one year compared with those discharged off anticoagulation. Readmission rates, including trauma-related readmissions, were similar between groups.<br /><br />The authors concluded that deciding whether to continue or stop anticoagulation after trauma is complex. Their findings suggest that restarting anticoagulation may reduce thromboembolic risk without clearly increasing overall major bleeding risk, though more data—especially on direct oral anticoagulants (DOACs)—are needed to guide clinical decision-making after traumatic injury.
