Post-Discharge Home Monitoring Programs for Patients With Covid-19: Results of a Survey of Hospitals in the Hospital Medicine Re-Engineering Network

A cross-sectional survey of hospital medicine leaders in the HOMERuN network examined post-discharge home monitoring programs for patients hospitalized with COVID-19 at U.S. academic medical centers. The study was motivated by concern that these patients remain at risk for clinical deterioration after discharge.<br /><br />In March 2021, surveys were sent via REDCap to 83 participants, one per hospital; 35 hospitals responded (42%). Among respondents, 22 of 35 hospitals (63%) reported having a post-discharge monitoring program, and most of these (73%) had been created specifically for COVID-19. Programs commonly collected physiologic data (80%), but in most cases patients manually reported the information rather than it being automatically uploaded.<br /><br />The survey found substantial variation across hospitals in patient eligibility criteria, the type of interface used by patients, escalation thresholds, and how care escalation was handled. The rapid creation of these programs likely contributed to limited integration with hospital electronic health records (EHRs). Follow-up suggested that few, if any, programs had undergone systematic evaluation of effectiveness.<br /><br />The authors concluded that home monitoring programs can be implemented quickly during a public health emergency, but fuller integration with monitoring devices and EHRs would likely improve usability and value. They also emphasized the need for more evidence on program efficacy and on the best design features, especially regarding escalation of care.<br /><br />Key limitations included restriction to academic medical centers in a single national network and a relatively low response rate, which may introduce response bias.
