Population Effects of Implementation of Sipps Practice Frameworks on Blood Pressure, A1c, and Emergent Healthcare Resource Utilization

This prospective study evaluated structured outpatient interventions for high-risk patients with diabetes and hypertension at an underserved clinic between March 2023 and March 2024. The goal was to determine whether multidisciplinary care could improve clinical outcomes and reduce emergency healthcare use.<br /><br />For diabetes, the program included nutrition and diabetes education, pharmacist support for medication adherence and regimen optimization, regular HbA1c monitoring, smoking cessation counseling, and statin assessment. For hypertension, interventions included home blood pressure monitoring devices, provider reeducation on updated workflows, lifestyle and nutrition counseling, and reinforcement of cardiovascular risk reduction strategies.<br /><br />A total of 490 patients were included: 250 with diabetes and 240 with hypertension. Among diabetic patients, HbA1c improved significantly over 12 months, dropping from 11.0% to 8.5%. Patients with private insurance had the largest improvement, though uninsured patients also showed meaningful gains. Emergency department discharges fell by 28%, and inpatient length of stay decreased by 61%, suggesting reduced healthcare utilization.<br /><br />Among hypertensive patients, those with severe hypertension showed significant reductions in both systolic and diastolic blood pressure, while stage 1 hypertension patients had modest diastolic improvement. No significant changes were seen in the pre-hypertension group. However, emergency department discharges and inpatient length of stay increased overall in this cohort, likely influenced by comorbidities and other confounding factors.<br /><br />An equity analysis found that payer status influenced diabetes outcomes, with privately insured patients generally doing better than government-insured or uninsured patients. In contrast, hypertension outcomes were similar across payer types.<br /><br />Overall, the study suggests that structured, multidisciplinary outpatient interventions can substantially improve glycemic and blood pressure control, especially for diabetes, and may reduce emergency resource use in high-risk populations.
