Point-of-Care Ultrasound in Perioperative Medical Management: A Systematic Review

This systematic review examined the role of point-of-care ultrasound (POCUS) in perioperative medical management for adult inpatients. The authors noted that POCUS is increasingly used by hospitalists, but there had been no comprehensive summary focused on perioperative care outside the ICU.<br /><br />The review included adult patients undergoing planned or completed surgical intervention in whom POCUS was used during evaluation and clinical outcomes were measured. Pediatric studies, outpatient settings, intra-operative POCUS use, non-peer-reviewed articles, letters, case reports, and non-English publications were excluded. Searches were performed on September 8, 2022 across Embase/Cochrane (Ovid), PubMed, Scopus, and Web of Science using terms related to point-of-care ultrasound, POCUS, adult, and perioperative care. Study quality was assessed with the Newcastle-Ottawa Scale, and all screening, extraction, and bias assessment steps were done independently by two reviewers with a third resolving disagreements.<br /><br />Findings showed that most of the existing literature comes from anesthesiology rather than hospital medicine. The strongest available evidence involved cardiopulmonary and gastric ultrasound. Cardiopulmonary POCUS was most useful for guiding management in postoperative ICU care, while gastric POCUS had the most evidence for helping determine safe surgical timing. However, the ultrasound findings and clinical outcomes varied widely between studies, preventing categorical pooling or meta-analysis.<br /><br />Overall, the authors concluded that hospitalists should focus on perioperative clinical questions that POCUS can answer reliably, especially in preoperative evaluation and in non-ICU postoperative care on hospital wards, where the environment is less controlled.
