Pilot Study of Peripheral Internal Jugular Catheters to Avoid Cvad

This pilot study evaluated “peripheral internal jugular” (PIJ) venous catheters as an alternative to central venous access devices (CVADs) for hospitalized patients with poor IV access. PIJs are short catheters placed into the internal jugular vein under ultrasound guidance, intended for patients who need short-term access but do not require medications that must be delivered centrally.<br /><br />A hospital medicine procedure service was trained in PIJ placement and used them at clinician discretion for appropriate patients. Between March 2021 and November 2022, 28 placement attempts were made and 27 were successful (96.4%) in 25 patients. Most patients were medical inpatients, and no procedural complications occurred.<br /><br />The PIJs remained in place for a median of 2 days. Overall catheter failure occurred in 11 of 27 cases (40.7%), usually due to occlusion or migration. Midline catheters performed better than angiocath introducers and peripheral IV catheters, with significantly fewer failures (23.5% vs. 70.0%, p=0.04). Importantly, 21 patients (78%) avoided needing a subsequent non-tunneled CVAD during the same encounter.<br /><br />The authors conclude that PIJ placement is feasible, safe, and may help hospitals reduce use of CVADs in selected patients, thereby avoiding the procedural risks and catheter-associated bloodstream infection risks linked to central lines.
