Pediatric Hospitalists as Educators: Teaching Methods on Family-Centered Rounds

This prospective observational study examined how pediatric hospitalists teach during family-centered rounds and whether teaching methods differ by experience level. Although effective teaching techniques are described in the literature, teaching on rounds is not always formally taught, and little prior research has quantified how often these techniques are used in practice.<br /><br />The study was conducted at a tertiary care pediatric hospital from July 2022 to July 2023. Pediatric hospital medicine faculty were eligible to participate. After a preliminary session introducing 11 formal teaching techniques, participants were observed during three rounding sessions each. Teaching teams included an attending, 2–4 residents, and up to 2 students. Observers recorded the type and frequency of teaching behaviors using a standardized guide. Participants were grouped by experience tier: T1 (6 years post-training), T2 (6–11 years), and T3 (more than 11 years).<br /><br />Among 31 of 48 eligible participants, 93 observations were completed across the three tiers. A total of 1,777 discrete teaching occurrences were identified, averaging 20.4 per rounding session over 2.6 hours and about 14–15 patients. Teaching was frequent and varied, with no significant differences in the number or type of teaching techniques used by tier of experience. The mean number of teaching occurrences was similar across tiers (T1 20.8, T2 17.8, T3 22.3; p=0.25).<br /><br />The most common techniques were Visual Pearls (87.1%), Learner-as-Teacher (84.9%), and Teaching the Team Through Teaching the Family (84.9%). Techniques involving both family and patient were among the most frequently used. Overall, the study showed that pediatric hospitalists use multiple teaching strategies on rounds despite limited formal training in efficient teaching. The authors suggest future work comparing learner and hospitalist perceptions of these methods to increase awareness of the education provided on rounds.
