This study examined written policies from 10 pediatric health systems that addressed the inpatient care of adult patients and assessed whether those policies included elements of healthcare transition (HCT). The systems included nine free-standing children’s hospitals and one nested children’s hospital. Policies were identified through convenience sampling in spring 2019.<br /><br />Reviewers used a rubric built from two sources: the Got Transition framework and an inpatient conceptual transition framework. The rubric used yes/no scoring for policy elements, and inter-rater reliability was measured by percent agreement. Overall reviewer agreement was high for most items (about 80%), but agreement was poor when judging whether a policy actually addressed HCT, and also lower for items tied specifically to HCT processes and pediatric-to-adult transfer.<br /><br />All 10 policies included age cutoffs for adult inpatient care, with thresholds ranging from 19 to 35 years. Age was the only policy element present in every policy. Only six rubric components appeared in at least half of the policies. Items grounded in the inpatient transition framework had better reliability than those derived from Got Transition, suggesting less consensus about how inpatient adult care fits into the transition process.<br /><br />The authors conclude that although pediatric hospitals continue to care for adult-aged patients, policies are inconsistent and often do not clearly address HCT. They recommend convening stakeholders to develop consensus on how inpatient adult care should be incorporated into pediatric-to-adult transition policies.
