Pediatric Hospital Medicine (Phm) Onboarding Across the United States, Current Practices and Their Perceived Quality and Efficacy From the Perspective of PHM Group Leaders

This study examined pediatric hospital medicine (PHM) onboarding practices across the United States and leaders’ perceptions of their quality and effectiveness. The authors note that strong onboarding can improve employee engagement, satisfaction, and retention, but there is little research on optimal onboarding for hospitalists, even as PHM workforce challenges may grow.<br /><br />To understand current practices, the researchers surveyed leaders of PHM groups using a questionnaire developed with expert input. The survey, distributed through the AAP Section of Hospital Medicine Division Leaders listserv, included multiple-choice and free-text questions about onboarding structure and perceived effectiveness. Responses were analyzed with descriptive statistics.<br /><br />Leaders from 37 distinct sites completed the survey. Of these, 35 programs reported having formal onboarding processes. Common features included shadowing, written onboarding guides, and clinical resources to help new hires become efficient. Shadowing was universal across the programs. Nearly all groups also emphasized opportunities for new physicians to work with experienced PHM clinicians. Leaders identified on-the-job orientation with experienced physicians as the strongest feature of “extremely effective” onboarding.<br /><br />Overall, group leaders were generally satisfied with the efficacy of their onboarding systems. The authors conclude that many PHM groups already have formal onboarding practices and that these programs often prioritize practical, experience-based support. These findings may help programs without formal onboarding develop initial processes. The authors plan to next study employee perspectives to build a more complete understanding of best practices in PHM onboarding.
