Patients With Renal Failure Are Prone to Hypoglycemia in Outpatient Observation Unit Resulting in Increased Length of Stay

This retrospective cohort study examined whether renal failure increases hypoglycemia risk and length of stay (LOS) among patients with type II diabetes admitted to an outpatient observation unit. The analysis included 46,201 patients treated from January 2021 to December 2023 in a large southwest U.S. health system; 34.5% had renal failure. The average age was 66.5 years, and 50.9% were female.<br /><br />Results showed that patients with renal failure were older and had a significantly higher incidence of hypoglycemia than those without renal failure (6.3% vs 2.6%). Hypoglycemia was associated with longer LOS in both groups. Among patients with renal failure, those who experienced hypoglycemia stayed longer than those who did not (30.9 vs 26.0 hours, p<0.0001). A similar pattern was seen in patients without renal failure (27.1 vs 25.0 hours, p<0.0001).<br /><br />The study suggests that reduced renal clearance in kidney injury may raise insulin levels and increase hypoglycemia risk, especially in insulin-treated patients. These findings highlight the need for individualized glycemic management based on renal function to reduce complications and shorten observation-unit stays.
