Pdf Summary
This document describes a novel point-of-care ultrasound (POCUS) curriculum for Pediatric Hospital Medicine (PHM) fellows, emphasizing that fellowship is an ideal time to build ultrasound skills. The authors note that POCUS is increasingly important in pediatric hospital medicine, but few PHM fellowships currently include formal POCUS training.

The proposed curriculum is a two-week elective for two fellows at a time. Its overall goal is for fellows to learn how to obtain and interpret basic bedside ultrasound images of the heart, lungs, abdomen, soft tissue, and vasculature, while recognizing common pathology. The curriculum is structured to combine self-study, didactics, supervised scanning, and independent practice.

Before the elective, fellows complete an online baseline knowledge exam two weeks prior and an ultrasound reading packet one week prior. During the elective, each day begins with a didactic session focused on a specific organ system, including discussion of normal anatomy and examples of pathology. This is followed by 1–2 hours of hands-on scanning with faculty, then 3–4 hours of independent scanning to reinforce skills.

Assessment includes online knowledge exams before the elective, immediately after, and again six months later to measure retention. Fellows also complete a hands-on timed final assessment to demonstrate competence in obtaining basic ultrasound views. In addition, each fellow develops a POCUS portfolio to be completed by the end of the year.

The authors conclude that a two-week POCUS elective can help PHM fellows improve patient care and teaching, particularly in resource-limited settings or when specialists are unavailable. They present this curriculum as a model for other PHM fellowship programs interested in expanding ultrasound training.
