Overall Response Rate and Mortality Associated With Triple vs Dual Therapies in Transplant-Ineligible Multiple Myeloma - A Meta-Analysis

This meta-analysis compared triplet versus dual therapy in transplant-ineligible multiple myeloma using Phase 3 clinical trials identified through a PRISMA-guided search. From 51 screened studies, 3 trials met inclusion criteria, representing 660 patients receiving triplet therapy and 717 receiving dual therapy.<br /><br />The analysis assessed overall survival (OS), progression-free survival (PFS), mortality, minimal residual disease (MRD) negativity, and overall response rate (RR). Triplet therapy showed significantly better disease response than dual therapy, with higher odds of MRD negativity (OR 2.74, 95% CI 1.21–6.23) and improved response rate (OR 2.68, 95% CI 2.16–3.32). These findings suggest that adding a third agent can deepen treatment response.<br /><br />For survival outcomes, no statistically significant differences were found in OS or PFS between the two treatment strategies. Mortality results were mixed: the crude analysis was not significant, but sensitivity analysis suggested lower mortality with triplet therapy (OR 0.57, 95% CI 0.42–0.76).<br /><br />Overall, the study concluded that triplet therapy improves response depth and MRD negativity in transplant-ineligible multiple myeloma, but clear survival advantages were not consistently demonstrated. The authors noted moderate risk of bias and some heterogeneity across studies.
