Outcomes Associated With Regular Inpatient Use of Gabapentin Among Patients With Chronic Obstructive Pulmonary Disease

This retrospective cohort study examined inpatient gabapentin use among 2.56 million adults hospitalized with COPD across 283 U.S. hospitals using the Vizient Clinical Database. The authors defined “regular” gabapentin use as administration on at least 50% of hospital days.<br /><br />Key findings:<br />- 20.5% of COPD inpatients received gabapentin during hospitalization.<br />- Among gabapentin users, 86.9% received it regularly on 50% or more of inpatient days.<br />- Compared with patients who did not receive regular gabapentin, users were younger, more often female, and had higher rates of opioid-related disorders, depression, chronic pain, neuropathic pain, and opioid exposure.<br /><br />In unadjusted analyses, regular gabapentin users had lower rates of delirium and pneumonia but higher rates of naloxone use, 30-day emergency department (ED) visits, and 30-day readmissions.<br /><br />After adjustment for demographics, comorbidities, and common indications for gabapentin:<br />- Delirium was slightly less common (aOR 0.92).<br />- Pneumonia was slightly more common (aOR 1.03).<br />- Opioid reversal agent administration was more common (aOR 1.20).<br />- Pressure ulcers were more common (aOR 1.19).<br />- 30-day ED visits were more common (aOR 1.19).<br />- 30-day readmissions were more common (aOR 1.25).<br />- Length of stay increased by 1.81%.<br />- Index hospitalization costs increased by 6.02%.<br /><br />The authors conclude that regular inpatient gabapentin use in patients with COPD is associated with greater inpatient resource use and worse post-acute outcomes. However, they note important limitations, including lack of gabapentin dose data, unknown pre-admission use, and possible confounding by indication. They recommend further causal-inference studies to better separate the effects of chronic gabapentin use from inpatient administration.
