Out of Sight, Out of Mind: Harnessing the Power of Post-Hospitalization Outcome Audit

The “Out of Sight, Out of Mind” (OOSOOM) conference is a resident-led, easy-to-implement forum designed to improve post-hospitalization outcome review, a process that rarely happens during residency despite its importance for refining future diagnostic and therapeutic decisions. Residents were given lists of patients they discharged within the prior six months and selected one case to present. Each session included four resident presentations, with presenters reviewing 5–10 patients using EMR data and, when needed, phone follow-up. Presentations focused on de-identified patient information, lessons learned, and how future practice might change, with special attention to outcomes that differed from discharge expectations.<br /><br />The project evaluated impact using retrospective pre/post surveys after the first four conferences and a follow-up survey 2–4 years later. Before OOSOOM, only 8% of residents reported reviewing post-hospitalization outcomes “often” or “always.” Immediately after the conference series, 41% said they would “probably” or “definitely” review outcomes. In follow-up after graduation, 45% of attendees reported reviewing outcomes “often” or “always” in practice, compared with 22% of non-attendees.<br /><br />The authors conclude that OOSOOM helps residents reflect on their practice while building a habit of auditing patient outcomes. It also supports experiential learning, uncovers opportunities for quality improvement projects, and differs from traditional morbidity and mortality conferences because it is not limited to adverse events or negative outcomes. Many residents instead presented diagnostic feedback cases, which were viewed as especially educational.
