Nt-Probnp and Long-Term Mortality Risk in Healthy Adults

This study examined whether NT-proBNP predicts long-term all-cause and cause-specific mortality in U.S. adults without known coronary artery disease or congestive heart failure. NT-proBNP is a biomarker released by the heart in response to stress and is commonly used in heart failure diagnosis, but its value as a mortality marker in generally healthy populations is less established.<br /><br />Researchers analyzed data from the continuous NHANES survey (1999–2004) linked to National Death Index records through December 31, 2019. The final sample included 12,933 adults, with a mean age of 45.8 years; 52.8% were women. NT-proBNP was categorized using a cutoff of 125 pg/mL. Survival analyses included Kaplan–Meier curves and Cox proportional hazards models adjusted for major confounders such as age, sex, race, BMI, smoking, alcohol use, hypertension, diabetes, income, education, and kidney function. Sensitivity analyses using competing-risk and parametric methods were also performed.<br /><br />Compared with survivors, participants who died were older, more likely to have NT-proBNP above 125 pg/mL, and had higher rates of hypertension, diabetes, smoking history, and lower estimated GFR. The Kaplan–Meier curves showed significantly worse survival among those with NT-proBNP ≥125 pg/mL (p<0.001).<br /><br />Overall, the study concludes that NT-proBNP is a strong predictor of both all-cause and cause-specific mortality in adults from the general population. This suggests NT-proBNP may have value beyond heart failure diagnosis as a long-term risk marker.
