Novel Interprofessional Education Experience: Improving Hospital to Skilled Nursing Transitions

This document describes a novel interprofessional education (IPE) experience developed through Duke’s TeleHOPE Program to improve hospital-to-skilled nursing facility (SNF) care transitions. TeleHOPE already holds weekly multidisciplinary teleconferences with SNFs to review mutual patients and identify safety gaps during transitions of care. Because Duke medical students and GME trainees had limited existing training in this area, the team created a week-long virtual learning experience focused on interprofessional collaboration and common barriers to safe hospital-to-SNF transitions.<br /><br />The goals were to evaluate learner reactions and assess changes in attitudes toward interprofessional care. The program used a mixed-methods post-survey, adapted from the W(e) Learn Interprofessional Program Assessment tool, along with open-ended questions. Across 18 sessions, 40 learners from three institutions participated, and 30 completed the survey.<br /><br />Results showed the experience was engaging, responsive to learner needs, and well received. Most participants reported a stronger appreciation for interprofessional care and increased motivation to provide better team-based care. Open-ended responses highlighted three main themes: appreciation for collaboration and communication, inspiration to improve discharge-related tasks, and new knowledge about what happens after patients transfer to an SNF. Learners noted the importance of clearly communicating roles, improving medication reconciliation and discharge summaries, and understanding how care gaps can occur during transitions.<br /><br />The authors conclude that a virtual interprofessional educational experience can improve learners’ knowledge, attitudes, and motivation related to care transitions. They suggest the model could be used elsewhere and plan to expand it to include other professions such as physical therapy and occupational therapy, while also studying longer-term outcomes.
