Nocturnist Perceptions of Night Medicine Education: A Multi-Institutional Survey

This multi-institutional study examined how nocturnists perceive their role as educators during night medicine rotations. Night rotations are common in internal medicine training and can improve resident quality of life, but they often offer fewer educational opportunities than daytime rotations. Because nocturnists are increasingly present in hospitals, they may help improve supervision and teaching overnight.<br /><br />The investigators surveyed nocturnists who work with learners at hospital/academic internal medicine programs. The web-based survey included two demographic questions and three 5-point Likert-scale questions. It was conducted from April to October 2023 and was IRB exempt. Out of 60 nocturnists surveyed, 38 responded, for a 63% response rate. Respondents represented institutions across 17 states.<br /><br />Results showed that nocturnists generally believed they could provide meaningful directed teaching at night: 53% said “often” or “always.” However, they also felt learners may be less receptive to teaching overnight, with only 11% saying “always” and 42% saying “sometimes.” In addition, few nocturnists reported using structured educational materials; about two-thirds said they “never” or “rarely” use them.<br /><br />The authors concluded that nocturnists perceive themselves as capable educators, but night learning may be limited by reduced learner receptiveness and limited use of structured teaching tools. They suggest that introducing structured educational materials could improve the quality and effectiveness of education on night rotations.
