New Nocturnist Training: Evaluation of a Pilot Faculty Development Initiative

This faculty development initiative aimed to improve the preparedness and comfort of new nocturnists as they began independently supervising resident-led rapid response teams and managing overnight clinical emergencies. The training addressed a common gap in hospital medicine: although hospitalists complete internal medicine residency, many early career nocturnists still feel unprepared for high-stakes situations without senior support.<br /><br />The pilot program consisted of a single day of hands-on instruction before clinical duties began. Each new nocturnist received one-on-one teaching from senior faculty on five practical emergency skills: intraosseous access, use of a ZOLL automated external defibrillator, ultrasound-guided IV placement, laryngeal mask airway placement, and bag-valve-mask ventilation. Participants also took part in simulated rapid response and code scenarios, rotating through team roles and applying the newly practiced skills in realistic clinical situations.<br /><br />After training, participants completed an anonymous REDCap survey rating their preparedness, comfort, and expected use of each skill on a 5-point Likert scale. Early results showed that before training, participants felt most prepared and comfortable with ultrasound-guided IV placement and bag-valve-mask ventilation. After training, the largest gains were seen in confidence and preparedness for ZOLL AED use. Ratings improved for all skills except bag-valve-mask ventilation, which remained high. In free-text responses, all participants described the five skills as likely, very, or highly applicable to nocturnist work.<br /><br />Overall, the findings suggest that this training model may help new nocturnists feel more prepared and confident in managing critical overnight events, and could be a useful ongoing part of faculty development and onboarding.
