Necrotizing soft tissue infections (NSTIs) are severe, rapidly progressive infections with high mortality, estimated at 20–40%. They can be caused by a variety of bacteria, including the relatively rare but potentially emerging pathogen Streptococcus dysgalactiae. Clinical presentation may vary, but extremity pain with any skin changes should raise suspicion for NSTI. Gastrointestinal symptoms are uncommon, but when present they may predict streptococcal NSTI development. Poor prognostic factors include a WBC count above 30,000/μL, creatinine of 2.0 mg/dL or higher, age over 60 years, and delayed surgical debridement beyond 24 hours.<br /><br />The excerpt also mentions platelet consumption and petechial rash as possible manifestations, reflecting systemic involvement in severe infection. Petechiae may indicate vascular injury, toxin-mediated effects, or coagulopathy associated with invasive streptococcal disease.<br /><br />Because NSTIs progress quickly and can be fatal, treatment must not be delayed.
