Nationwide Analysis of the Impact of COVID-19 on ECMO Utilization in Patients Hospitalized With STEMI: A Retrospective Observational Study

This retrospective national database study examined whether COVID-19 affected extracorporeal membrane oxygenation (ECMO) use among hospitalized patients with ST-segment-elevation myocardial infarction (STEMI) in the United States.<br /><br />Using Nationwide Inpatient Sample data from January to December 2020, the authors identified 198,335 STEMI hospitalizations after excluding patients under 18 and those with missing demographic data. Of these, 6,455 patients had concurrent COVID-19 and STEMI, while 191,880 had STEMI without COVID-19.<br /><br />Patients with STEMI and COVID-19 were more often female, Hispanic, African American, on Medicare, and from lower-income groups. ECMO use was more common in the raw comparison among STEMI patients with COVID-19 than those without COVID-19 (0.86% vs 0.07%). However, after adjusting for other factors in regression analysis, STEMI patients with COVID-19 had significantly lower odds of receiving ECMO than those without COVID-19 (OR 0.33, 95% CI 0.25–0.44, p<0.0001).<br /><br />Other factors associated with higher ECMO use included female sex, African American race, heart failure, and arrhythmias. The model showed strong predictive performance (AUC 0.929).<br /><br />The authors conclude that although ECMO may benefit some patients with severe COVID-19 and STEMI, real-world data suggest these patients are less likely to receive ECMO. They recommend further prospective studies to better evaluate ECMO’s role in severe COVID-19-associated STEMI and to help guide care.
