Moving From Burnout to Balance: Standardizing the Direct-Care Hospital Teaching Service for Faculty and Student Success

This document describes an initiative to improve Direct Care Hospital Teaching Services (DCHTS), where medical students are paired with hospital medicine faculty. As these services have expanded, faculty have struggled to balance being both the primary physician and sole educator, while students sometimes feel like burdens. The project aims to transform DCHTS into a more sustainable, high-value curriculum that better supports faculty and improves the student learning experience.<br /><br />The team conducted a six-month baseline assessment of both students and faculty using quantitative and qualitative methods. Based on those findings, they created a Direct Care Apprenticeship Model Curriculum (DCAM). Key features include a standardized weekly curriculum focused on specific entrustable professional activities (EPAs), an educational handoff tool to guide structured feedback, clinical reasoning teaching materials, and transparent scheduling. Faculty development was provided before implementation.<br /><br />After implementation, the team planned another six-month assessment using the same mixed methods approach. Preliminary results suggest that DCAM has a positive impact on both learners and faculty, but implementation was limited by clinical workload and other demands. The authors conclude that the model shows promise, but future efforts should focus on stronger faculty support and greater curricular uptake.
