Mortality Trends of Social and Demographic Disparities of Ischemic Heart Disease in Copd Patients Among Adult (>35 Years of Age): A CDC Analysis From 1999-2020

This CDC WONDER analysis examined mortality trends from ischemic heart disease (IHD) among U.S. adults aged 35 and older with chronic obstructive pulmonary disease (COPD) from 1999 to 2020. Using death certificate data, the study calculated age-adjusted mortality rates (AAMRs) and annual percent changes (APCs), stratified by year, sex, race/ethnicity, and geography.<br /><br />A total of 1,453,650 deaths were identified. Overall, IHD-related mortality in COPD patients declined from 1999 to 2018 (APC: -2.02%), then increased from 2018 to 2020 (APC: 4.51%). Men consistently had much higher mortality than women throughout the study period, with an overall AAMR of 55 compared with 27.1 in women. By the end of the study period, rates were 50.7 in men and 25.1 in women.<br /><br />Racial and ethnic disparities were also evident. White adults had the highest overall AAMR (42.8), followed by non-Hispanic American Indian/Alaska Native adults (37.9), non-Hispanic Black adults (27.5), Hispanic or Latino adults (19), and non-Hispanic Asian/Pacific Islander adults (11.2).<br /><br />Geographic variation was substantial. The highest mortality rates were seen in West Virginia, Vermont, Tennessee, Ohio, Oklahoma, and Kentucky, while the lowest were observed in Hawaii, Georgia, the District of Columbia, Massachusetts, and Utah. Regionally, the Midwest had the highest overall AAMR (43.1), followed by the South (40.3), West (34.8), and Northeast (34.4). Nonmetropolitan areas had higher mortality than metropolitan areas (51.3 vs. 35.9).<br /><br />Overall, the study found fluctuating mortality trends with significant demographic and geographic disparities, highlighting the need for targeted prevention and treatment strategies to reduce IHD-related deaths among COPD patients in the United States.
