Medical Trainee Knowledge and Practices Regarding Oral Medications for Inpatient Diabetes Management

This study examined whether internal medicine trainees’ knowledge matched their actual inpatient prescribing practices for oral diabetes medications, especially metformin. <strong>Background:</strong> Oral diabetes medications are generally safe to continue during hospitalization, but there is little information on how trainees manage them in practice. <strong>Methods:</strong> Trainees completed a pre-test on inpatient diabetes management before an educational intervention. <strong>Results:</strong> Most trainees believed some non-insulin diabetes medications could be safely used in the hospital. However, their reported practice did not align with this knowledge: more than half said they <strong>rarely or never</strong> continued home metformin at admission. The most common reason for hesitation was <strong>fear of lactic acidosis</strong>, followed by concerns about hospital culture, drug interactions, and hypoglycemia. <strong>Conclusions:</strong> Although trainees generally recognized that oral diabetes medications can be safe for inpatient use, they often did not continue them in practice. The findings suggest a need for educational interventions to address misconceptions, especially around metformin and its risk profile, and to improve alignment between knowledge and clinical behavior.
