Med Rec Mondays: Improving Medication Reconciliation Among Internal Medicine Residents

This quality improvement project, “Med Rec Mondays,” aimed to improve medication reconciliation (MR) documentation among internal medicine residents on general medicine teaching services (GMTS) at an academic urban hospital. MR is important for patient safety and transitions of care, but inconsistent use of the EHR led to duplicated work and low confidence in prior documentation.<br /><br />The team introduced a streamlined MR workflow, onboarding education for incoming interns, reinforcement at the start of each inpatient rotation, weekly color-coded dashboards sent via HIPAA-compliant message, and gamification through recognition and cookies for the top-performing team. The intervention ran from July 31 to November 3, 2023, and built on a prior project that had already improved MR completion through competition alone.<br /><br />Results showed meaningful improvement. In the three months before the intervention, the average MR completion rate was 39.5%. During the project, the average completion rate increased to 62.4%, reaching as high as 75% at one point. This increase was statistically significant (p < 0.0001).<br /><br />The authors concluded that combining education, simplified workflow, weekly feedback, and gamification successfully improved MR completion rates and reduced redundant work. They suggest that similar interventions could be implemented at other academic medical centers to support safer medication management and better continuity of care.
