Management of Inpatient Hyperglycemia in Non-Critical Settings

This quality improvement project addressed inpatient hyperglycemia management in non-critical care settings, where diabetes and new-onset hyperglycemia are common and associated with poor outcomes. The team introduced a uniform blood glucose protocol to improve safety and consistency while avoiding hypoglycemia.<br /><br />The project aimed to reduce hyperglycemic episodes by 20% within two months of implementation, without increasing hypoglycemic events. Data were collected for two months before and two months after protocol implementation, including 30 patients before and 22 after. Blood glucose readings were recorded at fasting, pre-lunch, pre-dinner, and bedtime, totaling 495 readings before and 553 after.<br /><br />Results showed a 27% decrease in hyperglycemic episodes after implementation, exceeding the original goal. Hypoglycemia remained low and unchanged, with only 4 episodes (1%) of blood glucose readings between 50–70 mg/dL both before and after the intervention.<br /><br />The authors concluded that poor inpatient glucose control can lead to worse outcomes, and that managing glucose in hospitalized patients is challenging due to concerns about hypoglycemia. They emphasized that standardized, institution-specific protocols and staff education are essential for achieving appropriate glycemic control. The project demonstrated that a structured protocol can significantly improve hyperglycemia management without increasing hypoglycemia.
